Admissions

Thank you for inquiring on behalf of your client and/or child about the services provided by Thompson Child & Family Focus. Applications for Residential Treatment, Day Treatment, Outpatient Services and Foster Care are available online. We also request that a Medical Exam Form, as well as the following information, accompany the application for the service you are requesting:

• Diagnostic Assessment and/or Psychosocial History
• Discharge Summaries from previous placements and/or hospitalizations
• Physical Exam performed by a physician within the last year
• Documentation of a TB Test done within the last year
• Psychological Evaluation, including IQ information
• List of all medications being used
• School Records (IEP) and/or 504 plan
• CON - included
• PCP or current Service Plan

The completed application can be mailed to:

or faxed to the attention of the Admissions Department at 704-531-9266.

Upon receiving a complete intake packet, including all of the above documents, our Admissions Committee will meet on the following Monday or Thursday to make a decision.
