Application Procedure for Thompson Child and Family Focus
Psychiatric Residential Treatment Facilities (PRTF)

Thank you for inquiring on behalf of your child/client.  Please complete the TCFF Application for Service and include the following documents:

• Comprehensive Clinical Assessment, completed within the past year
• Discharge Summaries from previous placements and/or hospitalizations
• Child's Health History Questionnaire(form available)
• Psychological Evaluation, including IQ information
• Current PCP
• School Records (IEP) and/or 504 Plan (if applicable)
• Service Application
• Checklist for PRTF

You may also include any other information you think would be helpful to our Admissions Committee in evaluating this child.  Additional paperwork/documents are required to process an admission, and will be requested if the application is approved.  Upon receiving a completed application packet, including all of the above documents, our Admissions Committee will review the information. I will contact you with their decision within 24 to 48 hours.
