This notice describes the practices of Thompson Child & Family Focus with respect to an individual’s Protected Health Information (“PHI”). Thompson is required by law to take reasonable efforts to ensure the privacy of an individual’s PHI. TCFF is also required to inform that person about its uses and disclosures, privacy rights, and Thompson’s duties all related to their PHI.
