Targeted Case Management is a Mental Health program providing planning and oversight for those needing community services ages 3 to 18. Case Management includes:
- Coordination and oversight of initial assessment and ongoing activities.
- Identification of strengths that will aid individuals in their recovery as well as the identification of barriers that impede the development of skills necessary for independent functioning in the community.
- Coordination of clinical services and community supports including discharge planning, community re-entry following hospitalization, residential services and other levels of care.
- Initial development, implementation and monitoring of each client's Person-Centered Plan, which includes medical and non-medical providers, the consumer, and natural and community supports.
- Direct interventions in situations that are escalating to prevent crisis, based on identification of cues and triggers.
- Children enrolled in the Case Management program must have a comprehensive clinical assessment to determine medical eligibility.
