Kaleb finds hope to help him out of crisis

Kaleb enjoys sports, fishing, and being outdoors. He dreams of going to trade school to become a welder. Kaleb lived with his grandparents while his mother struggled with substance use, and his father refused to provide care. His inability to regulate his emotions & recognize the impacts of his aggressive behavior led him into DHSS custody. He was placed in a group home but discharged due to safety concerns resulting in hospitalization. Kaleb was moved to another group home, was found with illegal substances, and found his roommate deceased.

 

Kaleb was at crisis point with no placement options until he arrived at our new, short-term crisis stabilization program. He is receiving individual therapy and participating in groups lead by the direct care staff. During this period of crisis stabilization, Kaleb will be assessed to determine the appropriate level of care he needs for successful long-term placement. We also undertake care coordination with outside stakeholders, and are constantly discharge planning so he can transition smoothly into the appropriate program for his needs.
