This is an enhanced therapeutic service eliminating out-of-home placements and higher levels of care by working with foster placements, adoptive homes, biological families, and kinship placements. The program includes the following:

approach

Interventions are delivered in the client's home and community. The home-based practitioner and the family share a uniquely close, intense and personal relationship.

goals

The program is committed to empowering families to set and achieve their own goals. Participatory assessments permit the family to be in charge of the process.

focus

Staff members carry small caseloads to permit more interaction with each family.

duration

Intensity and duration of services are customized for each family with a minimum of 6 months. Services include working with the community system to access local support resources.
