The service functions as a bridge between foster care, delivering care coordination and trauma-informed engagement during periods of placement instability or entry into state custody. Transitional Support includes:

24/7 supervision and support in home-like environments

Trauma-informed engagement grounded in CARE principles

Daily structure and life skills development to support readiness for next placement

Crisis de-escalation and behavioral support to prevent disruptions

Emotional and relational support to promote healing and trust-building

Coordination with case management and DCS to ensure continuity of care

Residential Case Management services are available across four strategically located regions in Tennessee: Memphis, Nashville, Lebanon, and Knoxville. This allows the program to serve youth throughout West, Middle, and East Tennessee.
